Who needs the Bridge to Home Rehabilitation Program?
The person who:

• Is recovering from major surgery, including joint replacement
• Has experienced repeated hospitalizations
• Is recovering from a stroke
• Is recuperating from a major accident
• Needs help managing pain
• Requires wound care or amputation care
• Is coping with congestive heart failure
• Is gradually getting better from a serious condition
• Needs enteral nutrition

To qualify for the Bridge to Home Rehabilitation Program, patient must:

• Be under the supervision of professional personnel
• Require skilled services on a daily basis
• Have a 3-day acute admission prior to entering the Bridge to Home Rehabilitation Program
