Hours: 8 a.m. - 4 p.m. seven (7) days per week.

The Catskill Regional Medical Center Adult Day Health Care Program provides medically-supervised services seven days a week for individuals with physical or mental impairments and challenges needing daytime assistance away from home. The program provides the medical and rehabilitative care, along with improving self-esteem through physical, mental and social activities. Our program covers a broad spectrum of needs and services providing families with quality care for their loved ones.

Our Team of Professionals Includes:

  • Medical director
  • Registered nurses
  • Dietitian
  • Social worker
  • Pharmacist
  • Certified nursing assistant
  • Social Worker: monitors and addresses psychological issues, registrant rights and HIPPA.

About the Adult Day Health Care Program

 

CRMC's Adult Day Health Care Program provides:

  • Continental breakfast, choice of hot or cold lunch and snacks
  • Wheelchair accessible transportation to and from our facility.
  • Coordination of visits to doctors
  • Diagnostic tests
  • Healthy living program

Nursing Services Include:

  • Initial and ongoing evaluations by Registered Nurses
  • Medication administration
  • Medication management
  • Monitor vital signs, weights
  • Monitor glucose and labs as per physicians orders
  • Personal care, assistance with ADLs and showers
  • Assisted or total feeding
  • Eternal tube feeding
  • Wound care
  • Respiratory treatments
  • Monthly health presentations
  • Ongoing health education
  • RNCP (Restorative Nursing Care Program)

Rehabilitation:

  • Physical Therapy
  • Occupational Therapy
  • Speech Therapy

Socialization and Recreational Activities Include:

  • Arts and crafts
  • Music therapy
  • Pet therapy
  • Mental awareness programs
  • BINGO
  • Ethnic and cultural activities
  • Exercise program
  • Birthday and holiday celebrations
  • Cooking groups

Nutrition Services Include:

  • Continental breakfast
  • Choice of hot or cold lunch
  • Healthy afternoon snacks
  • Kosher meals available upon request
  • Diet counseling and care planning

Case Management Includes:

  • Collaboration with outside agencies to promote healthy lifestyle
  • Assist with Medicaid recertification, food stamps, SSI, personal care services and Senior Housing
  • Case management for medical appointments coordinated with transportation if needed.

Rates:

Adult Day Health Care and transportation costs are covered by Medicaid, Managed Long Term Care and private pay rates available with and without transportation.

Insurance:

Managed Long Term Care Insurance and Medicaid are both accepted. Please call for further information.

The Long Term Care Unit at Catskill Regional Medical Center has been designated an Island Peer Review Organization (IPRO) Award-Winning Facility for providing the highest quality of care for individuals requiring both long-term care and short-term rehabilitation. This 64-bed facility is accredited by The Joint Commission.

We are dedicated to promoting the highest level of independence and the best medical care in a home-like setting. We offer both private and semi-private rooms with breathtaking views of the Catskill Mountains. Residents have spacious activity rooms, lounges, sunrooms and internet access. During the warmer months, residents are able to enjoy the beautiful Woodland Wildings Garden, located on the grounds for both residents and their family members.

The Long Term Care Unit also offers the distinct advantage of being on the campus of Catskill Regional Medical Center. In the event of a medical crisis, qualified medical experts are just a few steps away. In addition, a great emphasis has been placed on rehabilitation and successfully transitioning residents back in to the community. The therapy team provides resident specific programming right in the unit.

The health services provided at the Long Term Care Unit allow for complete access to the full range of medical services offered at the hospital- 24-hour skilled nursing care; rehabilitation services; physical, occupational, speech and respiratory therapies; podiatry services; dental care; and laboratory and radiology services.

Our Professional Team is comprised of: Medical Director, Licensed Nursing Home Administrator, Registered Nurses, Licensed Practical Nurses, Certified Nursing Assistants, Certified Social Worker, Certified Activities Director, ADA Registered Dieticians, Licensed Physical Therapists, Licensed Occupational Therapists, and Licensed Speech/Language Pathologists.
